Anyone who visits websites or performs transactions online needs to be aware of the dangers of identity theft. If you’re not careful, a simple slip can expose your personal information to thieves and lead to your identity being stolen. With your personal details, a thief can damage your credit, make it difficult to get loans, mess up your tax reporting, and steal from your bank accounts.

Identity theft can also impact your account at DTE. If you believe you are a victim of identity theft, please download the form described below, and then complete and submit the information so we can investigate your claim:


1. A notarized Federal Trade Commission Identity Theft Report Form, which can be obtained at https://www.identitytheft.gov.


2. A copy of a valid government-issued photo-identification card (i.e. valid driver’s license, state identification card or passport) both front and back.

3. Proof of residency for the time-frame the identity theft occurred (i.e. Rental/lease agreement, paystub, insurance document, tax form, bank statement).

4. A copy of the filed police report. If you are not able to obtain a copy, please provide the report number, date and location where the claim was filed, and officer’s name and contact information. A DTE Energy representative will contact you within 2-3 business days once all required documentation has been received to confirm receipt and provide further instructions if needed.
